2. Promo Period/Duration:
-October 1, 2011 – December 31, 2011
-Deadline of Submission of Entry – December 31, 2011 (Store Closing Hour)
* All entries must be received not later than December 31, 2011. Employees of NNDC, NNT and SMD and its agents will have no responsibility for lost, illegible, incomplete, mutilated and late entries.
* All entries must drop in drop boxes located at NNDC Multibrand Branches.
-Grand Draw will be on January 14, 2012
* The draw will be held at NNDC Main Office with the presence of a DTI Representative. The following prizes will be drawn:

3. List of Participating Branches:

4. Who are disqualified?
* Employees of NNDC, NNT and SMD, their affiliates, distributors, dealers, suppliers, advertising and promotional agencies as well as their relatives up to the second degree of consanguinity and affinity are not eligible to participate in this promotion.
5. By participating, the winners agree to the inclusion of their names and /or pictures in the Winners Ad.
6. A participant can only win once.
If the name is drawn more than once, the winner will receive the prize of a higher value and the winner for the other prize will be re-drawn.
7. Winners will be notified by telegram or telephone call as when and where they can claim their prizes.
8. Prizes are not transferable or convertible to cash.
9. In claming a prize, the winner must present the notification telegram plus 2 original and valid identification documents.

10. In case the winner fails to present original and valid identification documents, the winner should present the following:
* A sworn affidavit executed by two (2) disinterested persons attesting to his/her identity.
* Certification from the responsible Municipality/City official verifying that he/she is a resident of the said Municipality/City.
11. In cases where the winner, for justifiable reasons, cannot personally claim his/her prize, he/she can send a proxy on his/her behalf. The proxy should present a letter of authorization duly signed for by the winner. This should be submitted with the notification telegram sent by NNDC and two (2) original and valid identification documents of the winner. The proxy should also bring two (2) original and valid identification documents.
12. Prizes will be awarded to the person whose name appears on the winning entry coupon.
13. Prizes not claim within 45 days from notification of winners will be forfeited.
14. NNDC will not shoulder the final withholding taxes on prizes.
15. NNDC will not be liable to the winners for any taxes, costs, liabilities or expenses that may arise out of ownership or use of the prizes.
16. All decisions of NNDC are final and no objections to be entertained
